6 p.m.-7:30 p.m. at the Behavioral Health Services conference room, Garden Way, Hermitage

About Our Program

Families that face the death of a loved one are often unprepared for the emotional and physical stress associated with an illness of a limited life expectancy.

Sharon Regional’s Hospice and Palliative Care helps by providing support to family members as they participate in caring for their dying loved one.

Through visits, the Sharon Regional hospice team plans a program of care committed to keeping patients comfortable, maintaining the highest possible physical, emotional and spiritual quality of life. Pain and symptom control are a major focus of this plan. These efforts enable the terminally ill to remain home for as long as possible.

Sharon Regional’s Hospice and Palliative Care Program is licensed by the state Department of Health, is Medicare Certified and is accredited by the Joint Commission on Accreditation of Healthcare Organizations (JCAHO). It is a member of the PA Hospice Network.

Hospice and Palliative Care Is:

Provided wherever the patient resides

  • Physician directed
  • Nurse coordinated
  • Planned by a team of physicians, nurses, social workers, clergy, counselors, volunteers, and other professionals
  • Care that emphasizes symptom control
  • Covered by most insurances

Patients are accepted for Hospice Care regardless of sex, race, age, creed, national origin or ability to pay.

Memorial Donations

The Hospice Program gratefully accepts memorial donations. Donations enable Hospice to offer equipment and services to those in need.

Please include the name of the person you wish to honor. Checks can be made payable to Sharon Regional Health System Hospice Program and mailed to Hospice at 2320 Highland Rd., Hermitage, PA 16148. Third party checks can't be accepted.

Criteria for Admission

Referral by a licensed physician

  • Life expectancy measured in weeks or months rather than in years
  • Patient and family consent for hospice care
  • Residence in Mercer County or northern Lawrence County, PA.

Hospice and Palliative Care Services

  • Skilled nursing
  • Physical, speech and occupational therapy
  • Aide services
  • Specimen collection/delivery to laboratory
  • Nutritional counseling
  • Medical supplies and equipment
  • Volunteer support
  • Bereavement counseling
  • Support groups for adults and children; Bereavement Support Group meets 3rd Tuesday monthly, 6 p.m., Behavioral Health Services, Garden Way, Hermitage
  • Spiritual care
  • Homemaker service/respite care

Hospice and Palliative Care Volunteers

Trained volunteers are an integral part of the hospice team. Volunteers provide companionship, emotional support, run errands or assist the caregiver with simple nursing care. Volunteers also provide support for the family during the bereavement period.

Office volunteers perform necessary typing, filing, mailings or telephone tasks.

For more than 20 years the program has provided support to family members in the Shenango Valley and surrounding areas as they participate in caring for a dying loved one.

Sharon Regional and John XXIII Home partner on hospice services

Sharon Regional's Hospice and Palliative Care Program and John XXIII Home in Hermitage are observing their one year anniversary of providing hospice and palliative care services to residents of the facility.

The relationship began when members of the John XXIII administrative team responded to requests from residents and their family members to have an option available for terminally ill residents.
Although John XXIII had successfully provided end of life care for years, the staff recognized that a formalized hospice and palliative care program offered a level of expertise and knowledge using a team approach across multiple disciplines that would complement the care provided by the facility.

This team approach allows the hospice program to utilize specially trained physicians, pharmacists, nurses, dietitians, counselors, clergy, and social workers to meet the unique challenges that terminally ill patients and their families face.  These challenges include difficult symptom management issues, assistance with advanced care planning, understanding where patients are in the disease process along with expected outcomes, psychological distress, spiritual distress, helping patients define goals for their symptoms, helping with anticipatory grief before the death, and lastly bereavement care after a patient has died.
