General Information

The Transitional Care Unit is a service of Sharon Regional located on the 5th floor, east wing of the hospital, 740 E. State St., Sharon. The Unit is a 38-bed transitional care facility licensed by the Pennsylvania Department of Health and certified to provide skilled Medicare services.

The Unit functions as a transition center for patients just finishing the acute phase of a hospital stay who still need a level of skilled care to complete their recovery. Their average length of stay is less than 20 days with most remaining in our care for one to two weeks.

Following their stay on our Unit, patients will return home, or to an alterative care setting appropriate to their needs, such as long-term care, assisted living, or a rehabilitation center.

The Unit accepts patients without regard to race, color, sex, national origin, handicap, or religious creed.

Admission Information

Patients may be admitted to the Unit by their attending physician. Patients whose admitting physician is not a member of the Sharon Regional Medical Staff may be admitted by the Unit’s Medical Director or other Health System medical staff member.

Therapy/Parenteral Nutrition; Respiratory Therapy; Physical Therapy; Speech Therapy/Dysphagia Therapy; Recreational Therapy; Social Work/Discharge Planning; Specialized Services; Audiology; Psychological Services; Nutritional Counseling

Visiting Hours
There are no visiting restrictions for the Unit. It is beneficial to know if visitors are going to visit late in the evening, early in the morning or during the night.

Reimbursement
The Unit services are normally reimbursed by Medicare, Social Security, other commercial insurance companies, or through self-pay. The admission coordinator will be happy to discuss your financial concerns.
